Position: RN Case Manager
Pay Rate: Starting at $50.00

Job Summary:
The Nurse Case Manager is responsible for coordinating and delivering compassionate, holistic care to hospice patients and their caregivers. This role involves working collaboratively with physicians and an Interdisciplinary Team (IDT) to develop and implement individualized care plans that prioritize comfort, dignity, and quality of life.

Primary Responsibilities:
  • Collaborate with physicians and the IDT to assess, plan, implement, and evaluate patient care plans.
  • Provide direct supervision of Hospice Aides and guidance to caregivers.
  • Deliver skilled nursing care based on palliative principles and holistic philosophy.
  • Ensure care plans reflect medical necessity, measurable objectives, and reasonable goals.
  • Document all nursing and case management activities in accordance with HiHealthCare policies and hospice protocols.
  • Act as the primary nurse within the IDT framework, requesting support from team members as needed.
  • Monitor and report changes in patient conditions to the attending physician.
  • Participate in IDT conferences and contribute expertise to the team.
  • Provide end-of-day reports to the on-call RN and fulfill on-call duties as assigned.
  • Support quality and performance improvement initiatives directed by clinical leadership.
  • Engage in regular educational activities to enhance professional practice.
  • Travel to provide care at patient homes or contracted facilities.
  • Perform other duties as assigned by the Patient Care Manager or Director of Clinical Operations.
Knowledge, Skills, and Abilities:
  • Strong nursing judgment and the ability to plan and provide holistic care.
  • Ability to identify and address the psychosocial needs of caregivers in collaboration with the IDT.
  • Flexibility, organizational skills, and the ability to manage multiple priorities.
  • Effective communication skills for interactions with patients, families, and team members.
  • Proficiency in problem-solving, decision-making, and critical thinking.
Qualifications:
  • Current Hawaii RN license in good standing; BSN preferred.
  • At least one year of experience as a staff nurse in an acute care setting; hospice or home health experience preferred.
  • Valid driver’s license and insured, registered personal vehicle meeting Hawaii safety standards.
  • Current TB clearance.
Compensation: $50.00 - $53.00 per hour
