This role is essential in coordinating and delivering high-quality, holistic care to patients and their families during end-of-life. The Nurse Case Manager works collaboratively with physicians and the Interdisciplinary Team (IDT) to create individualized care plans that focus on comfort, dignity, and quality of life.

Primary Responsibilities:
- Collaborate with physicians and the IDT to assess, develop, implement, and evaluate patient care plans.
- Provide skilled nursing care grounded in palliative principles and holistic philosophy.
- Supervise Hospice Aides and provide support and guidance to family caregivers.
- Ensure care plans are medically appropriate, goal-oriented, and clearly documented.
- Serve as the primary nurse for assigned patients, coordinating care within the IDT framework.
- Monitor and promptly report changes in patient conditions to the attending physician.
- Participate actively in IDT meetings, sharing clinical insights and recommendations.
- Deliver timely end-of-day handoffs to the on-call RN and fulfill on-call responsibilities as scheduled.
- Support quality improvement initiatives as directed by clinical leadership.
- Engage in ongoing professional education to maintain and enhance clinical skills.
- Travel to patient homes or contracted facilities as needed to provide care.
- Perform additional duties as assigned by the Patient Care Manager or Director of Clinical Operations.
Required Knowledge, Skills, and Abilities:
- Strong clinical judgment with a focus on holistic, patient-centered care.
- Ability to recognize and address the psychosocial needs of both patients and their caregivers.
- Excellent organizational skills and the ability to manage multiple priorities.
- Clear and compassionate communication with patients, families, and team members.
- Skilled in decision-making, critical thinking, and problem resolution.
Qualifications:
- Current Registered Nurse (RN) license in the State of Hawaii; BSN preferred.
- Minimum one (1) year of experience as a staff nurse in an acute care or clinical setting is required.
- (Hospice or home health experience is a plus but not required.)
- Valid Hawaii driver’s license and access to a registered, insured vehicle that meets state safety standards.
- Current TB clearance.
Compensation: $50.00 - $53.00 per hour
